Evolent Health acquired Vital Decisions, a New Jersey–based provider of technology-enabled advance care‑planning and telehealth services, from WindRose Health Investors. Vital Decisions will be folded into Evolent's specialty management offering, New Century Health, and consolidated into Evolent's Clinical Solutions segment to expand its specialty care and advance care‑planning capabilities.
