Affiliates of Signify Health and Remedy Partners, both portfolio companies of New Mountain Capital, signed a definitive agreement to merge and create a market-leading value-based care enablement platform. The combined business will unite Signify's national in-home clinician network with Remedy's bundled-payment software and analytics, creating a platform serving payers, providers and life sciences with integrated clinical, behavioral and social data.
