Jordan Healthcare, a leading Texas home health provider, has acquired Dallas-based Chartwell Home Care. Dallas-based private equity firm Trinity Hunt Partners invested approximately $16 million in Jordan Healthcare to support the acquisition, creating one of the largest regional Medicaid and Medicare home care providers in Texas.
