PurposeCare Acquires Home Sweet Home In-Home Care

PurposeCare, a Lorient Capital-backed home health platform, has acquired Home Sweet Home In-Home Care, a St. Joseph, Michigan-based home health agency founded in 2011. The deal expands PurposeCare's Midwest footprint and adds roughly 400 patients and more than 300 caregivers to its platform as it consolidates several regional agencies under one brand.

HomeCentris Healthcare Acquires Family First Home Healthcare

Owings Mills-based HomeCentris Healthcare has acquired Philadelphia-based Family First Home Healthcare, closing the deal on April 3. The acquisition expands HomeCentris' footprint into Pennsylvania and strengthens its ability to serve Medicaid and managed-care partners while integrating Family First's staff and operations into HomeCentris' technology and operational platform.

Family Resource Home Care Acquires Companion Care, Inc.

Family Resource Home Care, a portfolio company of Great Point Partners, has completed the acquisition of Companion Care, Inc., a Woodinville, Washington-based non-medical in-home care agency. The acquisition expands Family Resource's footprint to seven locations in the greater Puget Sound region and 29 locations across Idaho, Oregon and Washington, bringing the combined organization to over 1,700 home care professionals; Companion Care's ~100 employees have been offered roles with Family Resource.

Creach Family Holdings Acquires Freudenthal Home-Based Healthcare

Creach Family Holdings, a privately held family investment firm, has acquired Freudenthal Home-Based Healthcare, a St. Joseph, Missouri provider of in-home assistance, skilled nursing and hospice services. Freudenthal will continue to operate under its existing name and join CFH's Home Health and Hospice group (which includes Faith Home Health and Hospice), expanding CFH's in-home care footprint in the Midwest.

Humana Acquires onehome

Humana Inc. has signed a definitive agreement to acquire onehome (One Homecare Solutions) from WayPoint Capital Partners, accelerating Humana's development of a value-based home health offering and complementing its Kindred at Home integration. onehome, headquartered in Miramar, Florida, provides home-based services including home health, infusion, and DME and has experience with risk-based contracting in Florida and Texas; financial terms were not disclosed.

HouseWorks Acquires AccordCare's Connecticut Personal Care Division
