Grant Avenue Capital Acquires Heartland Rehabilitation and MileStone from HCR ManorCare

Grant Avenue Capital acquired the Heartland Rehabilitation and MileStone businesses from HCR ManorCare and combined them under the new brand H2 Health. The deal (terms not disclosed) creates a platform that operates more than 60 outpatient rehabilitation facilities across multiple states and a national clinical staffing business; Kiawah River Health Partners co-invested in the transaction.
