CenterWell, the healthcare services division of Humana Inc., completed its acquisition of Florida-based primary care platform MaxHealth from Arsenal Capital Partners and MaxHealth's founder-shareholders. MaxHealth operates ~58 owned clinics and 24 affiliated clinics (sources report 54 owned + 4 specialty clinics + 24 affiliates), is supported by roughly 530 employees and cares for more than 120,000 patients (including 80,000+ in value-based care programs). The acquisition expands CenterWell Senior Primary Care's senior-focused, value-based primary care footprint in key Florida markets; financial terms were not disclosed.

Palm Medical Centers to Acquire Trinity Family Health Care Center

Palm Medical Centers, a portfolio company of MBF Healthcare Partners II, has reached an agreement to purchase the assets of Trinity Family Health Care Center from Dr. Saurinkumar Shah. American Healthcare REIT Acquires Remaining 24% Stake in Trilogy REIT Holdings

American Healthcare REIT, Inc. (AHR) exercised its purchase option and acquired the remaining 24% minority membership interest in Trilogy REIT Holdings, LLC for approximately $258 million, becoming the sole owner of Trilogy REIT Holdings and its Integrated Senior Health Campuses (ISHC). The acquisition was funded with net proceeds from a concurrent equity offering, with remaining proceeds used to repay roughly $194 million of outstanding credit facility debt.

Kindred at Home operates home health, hospice and personal care services across ~40 states (serving ~550,000 patients annually and employing ~43,000 caregivers) and will be integrated into Humana’s Home Solutions/CenterWell home health platform to accelerate value-based, home-based clinical care.
